New Clients/New Patients:
- Medical history for your pet: i.e. vaccines, blood work, ECT.
- Bring a fresh fecal and urine sample.
- Current diet and any medications or supplements your pet is on.
- You can fax us pet history from another veterinary hospital at 724-443-6531.
Diarrhea:
- Bring a fresh stool sample.
- You can also keep a stool sample in the refrigerator over night if needed
- Any home treatments you have tried

Urinary problems:
- Bring a fresh urine sample with any type of urination problem.
- If you can’t get one do not allow your pet to urinate prior to the exam and a technician can help you get one at the office.

Semi-annual wellness exam - Information Needed:
- Type of food your pet is eating.
- Current medications or supplements.
- Vaccine history they might have received elsewhere.
- Any problems your pet might be having.
Skin and/or infections/itchy (Dermatitis):
- A technician will obtain cytology samples from your pet for microscopic examination to aid in the diagnosis of the infection.
- Please do not bathe the pet before you come in or clean the ears.
Sick visit - Please bring all history about your pet in the last few days:
- Any symptoms you pet is having and when they started
- How much your has your pet been eating and drinking
- If you pet has any vomiting and diarrhea
- If you pet has or could have gotten into anything in the last week or so
- If you know or suspect your pet has gotten into something like rat poison please bring the box or container it came in.
- Any medications your pet is on
